Records Requests: Each client is given a copy of their evaluations at the follow up appointment. Hardcopy records are no longer available. An electronic copy on USB can be made and mailed via USPS certified mail for $35 if needed after the follow up appointment.

Per Office Policy, Payment for services is due when signing in for appointments. Please do NOT ask for accommodations in paying these mandatory fees.

In- office appointments can be scheduled at the number below.

Please arrive 10-15 minutes early for your appointment to complete any required paperwork or make payments prior to your session starting. If you do not have the Forms filled out ahead of time, we recommend you arrive 30-45 minutes early to complete the forms. Be aware that your initial session will start on time and if the forms are not completed, that will reduce the one-on-one time you have with Dr. Davenport. If a second interview session is necessary to complete the initial interview, an additional fee of $175 will be billed.

We request that you download the Patient Forms (Blue button above) and fill them out ahead of time. They are .pdf fillable or you can print and fill in manually. Thank you in advance for filling out the forms before arriving for your appointment.

DUE TO SPACE AVAILABILITY, AND PRIVACY CONCERNS, WE REQUEST THAT ONLY THE PATIENT, OR PATIENT AND PARENT(S), ATTEND THE SESSION. WE UNDERSTAND THIS MAY CAUSE INCONVENIENCE IN SOME CASES, BUT WE MUST PROTECT PATIENT/CLIENT PRIVACY FOR ALL OUR CLIENTS TO THE MAXIMUM EXTENT POSSIBLE. Office sessions with Dr. Davenport are 45 minutes.

Calls are returned at the end of the day or in the early morning. 

*Once an appointment is scheduled, you will be expected to pay for it unless you provide 24 hours advanced notice of cancellation.

Patients are responsible for keeping their scheduled appointments. 

The initial fee of $250 will be forfeited if the initial session is cancelled within 24 hours of the appointment time or the appointment is missed.
