Scheduling, Rates and Office Policies

In-office and telehealth appointments can be scheduled at (972) 640-8026.

We request that you download the Patient Forms (Blue button above) and fill them out ahead of time. They are .pdf fillable or you can print and fill in manually. Thank you in advance for filling out the forms before arriving for your appointment. You will need to email the forms to Dr. Davenport, or print and bring into the office.

Per Office Policy, payment for services is due at the beginning of the appointment.

Psychotherapy Rates

$250.00 - Initial Intake Session

$200.00 - Individual Therapy/Follow-up

Any request for forms, letters, records, or documents (including paperwork for disability and legal matters) not directly related to your current or past treatment will be billed at a rate of $200/hour in six-minute increments.

Assessment Rates

Psychological assessments are determined by the specific diagnostic questions in order to provide a thorough and accurate measure of the client’s functioning. There are additional factors which impact the number of number of tests administered, clinical interviews and feedback, review of records, and/or consultation with other professionals.

All psychological assessments include a full battery including (but not limited to):

Intellectual Functioning

Academic Functioning – more measures are often needed to determine a Specific Learning Disability.

Visual-Motor Perceptual Reasoning Skills

Social and Emotional Functioning

Areas of Specificity including:

Attention-Deficit/Hyperactivity Disorder for both children and adults

Autism Spectrum Disorder for both children and adults

Mood Disorders

Anxiety and Obsessive-Compulsive Disorders

The average range of price for a psychological assessment is $2500 - $3500 based on the diagnostic criteria being evaluated. The actual cost will be provided prior to scheduling any assessments.

While we are unable to offer insurance billing, a detailed receipt will be provided upon request. It is important to know that if filing with an insurance company, your confidentiality is not guaranteed as a clinical diagnosis and clinical notes/psychological evaluations are often necessary to provide.

Please be aware that you are responsible for the full payment.

Cancellation Policy

Once an appointment is scheduled, that time is reserved specifically for you. If you are unable to attend a session, contact your provider at least 48-hours in advance in order to cancel without a charge. For all missed sessions, the Full Fee of your service will be billed. Dr. Davenport is only in the office for scheduled sessions.
