Existing Clients:
If you have not completed the forms recently, please complete the updated forms on the Forms page, print, and and bring in. This ensures we have the most current information on file.

Note: Psychological Offices - Texas is a cash/credit based business. We no longer accept insurance as a payment method. An initial non-refundable payment of $250 is required when scheduling psychological evaluations. The remainder is due upon arrival for the initial interview.
