Scheduling Policy

Appointments may be scheduled by calling the office at (972) 640-8026 or emailing Dr.SusanFDavenport@outlook.com.

New clients are required to:

• Complete all intake forms before the first appointment.

• Return paperwork via email or bring printed copies to the office.

Availability is limited, and appointments are reserved in advance. If Dr. Davenport is unavailable or out of the office, she will return calls or emails at her earliest convenience during regular business hours.
